1 <br />' 2018 Local Motor Fuel Tax <br /> Street Improvement Project <br /> City Project 2018-25 <br /> April 23, 2018 <br />' ADDENDUM NO. 1 <br /> TO PLANS, SPECIFICATIONS AND CONTRACT DOCUMENTS FOR: <br /> 2018 LOCAL MOTOR FUEL TAX <br /> STREET IMPROVEMENT PROJECT <br /> CITY PROJECT 2018-25 <br />' DATED APRIL 23,2018 <br /> AD-1-1 NOTICE <br />' 1.01 This Addendum No. 1 dated April 23,2018,to the Plans, <br /> Specifications and Contract Documents for the above captioned <br /> project, supersedes all contrary and conflicting information of the <br /> above mentioned bid proposal documents, which are hereby <br /> supplemented in certain particulars as follows: <br />' AD-1-2 BID PROPOSAL <br /> 2.01 Insert the attached pre-bid meeting sign-in sheet and meeting <br />' minutes from the Pre-Bid Meeting on April 18,2018. <br /> 2.02 Replace the Subcontractor Utilization Statement on Page 22 with <br /> the pages included with this addendum. <br />' AD-1-3 SPECIAL PROVISIONS AND STANDARD SPECIFICATIONS <br />' 3.01 Insert the following Special Provision, SP-19, into the Special <br /> Provisions and Standard Specifications on page 61. <br /> SP-19 SIDEWALKS, CURBS,AND CURB RAMPS <br /> Description: All sidewalks,and curb ramps must be replaced and open to <br /> pedestrian traffic within 7 calendar days of removal. <br /> Basis of Payment: This work will not be paid for separately but shall be <br /> considered as included with the various work items involved. <br />' Please make the proper acknowledgment of addenda#1 on page 15 of your <br /> bidding documents. <br /> END OF ADDENDUM NO. 1 <br /> d 3d <br /> City En ineer: � Date: v� <br />
