<br /> Decatur property, 100 block of South Oatsland.Avenue, for Lock, Stocic & Barrel Millikin
<br /> y Homecoming Party on the following date aad time:
<br /> Friday, pctober 5,201 S 5:00 p.m.— 12:00 a,m.(midnight)
<br /> by the sponsoring agency,its officers,agents,employees, and registrants,
<br /> Furfhermore, the sponsoring agency agxees to provide the City of Decaiur evidence of third party
<br /> liability insurance coverage for the event in aza amount not less than $1,000,000 per occurrence,
<br /> and$2,000,000 aggregate,for properry damage,and personal and bod.ily injury,including deafh.
<br /> Tf pe�mission is granted for this event,evidence of liquox liability insurance coverage in an amount
<br /> not less than$1,000,000 per occurrence and aggtegate must a�so be provided. The City of Decattu, ,
<br /> Illinois, must be named as additional inswred on both poliraes for the duration of the Bves�t.
