6. Time of Submission• Certificate of Insurance At ar before the Eime of executian of this <br /> agreement and prior to commencing any work activity an the project,the Consulting Engineer <br /> shall provide the City's Representative with certificates of insurance showing evidence the <br /> insurance policies noted below are in full force and effect. Cansulting Engineer shall give the <br /> City's Representative at least 30 days written notice prior to any material change,cancellation,4r <br /> non-ranewal except in the case of cancellation far non-payment of premiarn, in which case natice <br /> shall be 10 days. The certificates shall be attached hereto as Exhibit E.The Consulting Engineer <br /> shal3 provide any renewal certificates of insarance autamatically to the Cifiy's Representative at <br /> least 30 days priar to policy expiratian. The certi�cate tnust certify the fallowing: <br /> a. I'+tame and address of party insured. <br /> b. Name(s)of insurance company ar companies, <br /> c. Name and address of authorized agent executing such certificate. <br /> d. Description of type of ins�zrance and coverage afforded thereunder. <br /> e. Insurance policy numbers. <br /> f. Limits o�liability of such policies and date af expiration of policies. <br /> g. To the extent the same is available, insarance comp���y or companies shall further certify that <br /> said policies shall nat be modified,canceiled or terminated until after written notice to the <br /> City's Representative per standard ISO accord forrn wording and the palicy provisians. <br /> �. Types and Limits of Insurance. The Consulting Engineer shall provide the following: <br /> a. Workers`Con�pensation: <br /> Coverage A: Statutary Limits <br /> Coverage B: C?ne hundred thousand dollars($100,000)employer's I'rability limits far <br /> each accident or per disease,per employee. Said policies shall be endorsed Eo cover any <br /> disability benefits or Federal compensatian acts if applicable. <br /> 6. Generai Liabiiity: Combined single limits of,no less than,ane nailtion dollars($1,400,OOQ) <br /> per occun-ence/annual aggregate amount of two million dollars ($2,000,000). General <br /> Liability Insurance shali include: <br /> Personal Injury Liability coverage. <br /> c. Automobile Liability:Cflmbined si�gle lirnits of,no Iess than,one million dollars <br /> ($I,d00,00d)per occurrence/annual aggregate amount of two million dollazs($2,OQQ,000). <br /> Aato liability sha11 include hired and nan-owned autos. <br /> d. Prafessxanal Liability: A professional liability errors and ornissions policy with linnits of,no <br /> tess than,one million doilars{$1,000,000}per claimlannual aggregate amaunt af twa million <br /> dollars($2,000,000). If said policy is written on a claims made basis,the retraactive date of <br /> the policy�nust predate the date of this agreement. In additian,the policy term must extend <br /> one year beyond completian date of this agreernent. <br /> e. Self-insured: If a self-insured retention or dedactible is maintained on any of the policies,the <br /> Consulting Engineer shall provide the amount of the self-insured retention or deductible to <br /> the City, Such deductibles shall be subject ta approval by the City.
