2019 Annual Water Main Replacement Project <br /> City Project 2019-10 <br /> February 12,2019 <br /> ADDENDUM 1�0. 1 <br /> TO PLANS,SPECIFICATIONS AND CONTRACT DOCUMENTS FOR: <br /> 2019 ANNUAL WATER MAIN REPLACEMENT PROJECT <br /> FAIRLAWN NEIGHBORHOOD <br /> CITY PROJECT 2019-10 <br /> DATED JANUARY 2019 <br /> AD-1-1 NOTICE <br /> 1.O 1 This Addendum No. 1 dated February 1 Z,2019, to the Pians, <br /> Specifications and Contract Documents for the above captioned <br /> project, supersedes all contrary and conflicting information of the <br /> above-mentioned bid proposal documents,which are hereby <br /> supplemented in certain particulars as follows: <br /> AD-1-2 BID PROPOSAL <br /> 2.01 Insert the attached pre-bid meeting sign-in sheet and meeting minutes <br /> from the Pre-Bid Meeting on February 8, 2019. <br /> 2.02 Replace the Bid Proposal Forms on pages 17-19 with the Add No. I <br /> 17-Add No. 1 19 included with this addendum. <br /> AD-1-3 SPECIAL PROVISIONS <br /> 3.01 Insert SP-21 WATER MAIN TO BE FILLED, 6-INCH in the index on <br /> page 56 <br /> 3.02 Insert SP 21- WATERMAIN TO BE FILLED, 6-INCH on page 64 of <br /> the Special Provisions as described below: <br /> SP-21 WATERMAIN TO BE FILLED <br /> Description: This work will consist of filling the existing water <br /> main with a Controlled Low-Strength Material (CLSM) at the <br /> locations as shown on the plans and directed by the Engineer. <br /> Watermain to be filled shall be bullheaded and vented on the upper <br /> end(s). CLSM shall be placed fram the lower end to the upper end(s) <br /> until filled. Placement shall be pumped; gravity fed is not allowed. <br /> All cost to excavate, cut, and cap main ends for grouting shall be <br /> included in CUT AND CAP EXISTING MAIN <br />
