N w �Ria q <br /> Section 1. That the recommendation of the Decatur City Plan Commission be, and the <br /> same are hereby, received, placed on file and granted. <br /> Section 2. That upon compliance with the conditions described herein, and in the site plan <br /> attached hereto and hereby made a part hereof as Exhibit A, said herein that a Conditional Use <br /> Permit be, and it is hereby, granted to FANNIE COLE as the petitioner, and only to allow for a <br /> Day Care Home, Large in an existing residence, proposed to be consolidated with another single- <br /> family residence zoned R-3 Single-Family Residence District. <br /> 1. A maximum of sixteen(16) children per shift are allowed. <br /> 2. The hours of operation are Monday through Friday 6:00 a.m. to 12:00 a.m. <br /> 3. A maximum of three (3) employees are allowed. <br /> 4. A minimum of three (3) off-street parking spaces are required, in addition to the two (2)in <br /> the proposed garage. <br /> 5. A new fence six (6) feet in height is required within the rear yard after the garage and <br /> breezeways are constructed, as approved by the Economic and Community Development <br /> Department. <br /> 6. A final plat is required to be submitted and recorded to combine the two lots within the <br /> subject site. The property cannot be further subdivided after the construction is completed. <br /> 7. Approval of a site plan is required before building permits are to be submitted. <br /> 8. Architectural plans need to be approved before construction takes place. <br /> 9. Five(5)shrubs of no more three(3)feet in height are required as approved by the Economic <br /> and Community Development Department. <br /> Section 3. That said Conditional Use Permit granted hereby, be, and it is hereby,made <br /> subject to the applicable provisions and requirements of Ordinance No. 3512, as amended, the <br /> same being the Zoning Ordinance of the City of Decatur, Illinois, and that the Zoning Map of <br /> said City be amended to show the granting and existence of this Conditional Use Permit. <br /> PRESENTED, PASSED, APPROVED AND RECORDED this 19th day of August, 2019. <br /> � <br /> JULIE MOORE WOLF MAYOR <br /> ATTEST: <br /> � <br /> KIM ALTHOFF, CITY CLERK <br />
