2.2 Member Enrollment. Members will be enrolled in the Paladina Clinic upon the <br /> receipt by Paladina of eligibility census information from Client. Client shall provide Paladina <br /> with the eligibility census file at least 45 days prior to the Clinic Opening Date,and then on-going <br /> on a monthly basis. During the Term of this Agreement and subject to Paladina Clinic policies, <br /> an enrolled Member will remain enrolled in the Paladina Clinic until Paladina receives from Client <br /> notice that a Member has disenrolled for any reason and an Eligibility File that does not include <br /> such Member's information.Any new Member will become enrolled in the Paladina Clinic on the <br /> date that Paladina receives from Client an Eligibility File that includes such Member's <br /> information. <br /> 3. Paladina Responsibilities. <br /> 3.1 Paladina Services. Paladina shall be responsible for obtaining, providing, or <br /> otherwise making available,on its own behalf or through contractors, all of the services provided <br /> at the Paladina Clinic. Paladina shall ensure that Members enrolled in the Paladina Clinic have <br /> access to the following health and wellness services at the Paladina Clinic: <br /> (a) Primary health care services, including non-emergency episodic care and <br /> preventive care for all enrolled Members; <br /> (b) Electronic medical records and a Member portal to view such records; <br /> (c) Health coaching; <br /> (d) Tailored care plans for adult Members; <br /> (e) Health education for Members; <br /> (f) Standard On-Site Lab Services and Pharmaceutical Supplies described in <br /> Exhibit E; <br /> (g) Periodic identification of gaps in care, patterns of disease and trends in <br /> Member adherence across the Client's eligible population <br /> (h) Basic occupational health services, as indicated in Exhibit C, including: <br /> (i) Contact dermatitis; <br /> 0) Vision screening; <br /> (k) Pulmonary and Audiometry tests; <br /> (1) Initial injury evaluation; <br /> (m) Drug/alcohol screening; <br /> (n) Pre-employment and fitness for duty physical exams; and <br /> (o) Applicable portions of fire fighter physical exams. <br /> 3 <br />
