Agreement No 20-488039 <br /> EXHIBIT B <br /> DELIVERABLES OR MILESTONES <br /> The Grantee will provide the following to Grantor with its monthly reports as set forth in Article XL,herein: <br /> • A description of the progress of each of the tasks and timelines set forth on Exhibit A,herein. <br /> • A description of all outreach efforts to businesses during the month. <br /> • A list of applicants for the ES Program for the month being reported, identifying the type,address <br /> and county of the businesses. <br /> • A complete list of the business participants selected to receive financial support,as well as the <br /> associated financial support amounts and the type of support(grant,subsidy,expense <br /> reimbursement).The list should include the type,address and county location of the business <br /> participants. <br /> • The types of business participant categories Grantee will be asked to report on include the <br /> following: <br /> • Eventvenues <br /> • Music venues <br /> • Performing Arts Venues <br /> • Indoor Recreation <br /> • Amusement Park <br /> • Movie Theater <br /> • Museum <br /> • Charter/Shuttle Bus <br /> • Independently-owned retail <br /> • Restaurant,bar,or tavern <br /> • Fitness center <br /> • Tourism-and travel-related activities <br /> • Support services for arts and events <br /> • The basis for not selecting the business applicants denied financial support. <br /> • Copies of each executed certification or application or agreement containing a certification <br /> between the Grantee and the business participants for which the Grantee is seeking <br /> reimbursement. <br /> • Proof of payment to the business participants. <br /> • A ledger outlining the details claimed by the business participants as the"cost of business <br /> interruption."The Grantor will provide the ledger template to the Grantee. <br /> • For financial support payments made by Grantee to business participants after December 30, <br /> 2020,Grantee shall provide to Grantor with its request for reimbursement support documenting <br /> that the business participants'costs were incurred by December 30,2020. <br /> State of Illinois <br /> INTER-GOVERNMENTAL GRANT AGREEMENT FISCAL YEAR 2021/2 2 20 <br /> Page 32 of 49 <br />
