(d). Evaluation of Work Record.
The work record scoring for this examination shall be based on an average of the
employee's three(3)most recent recently completed full-year Civil Service Annual Performance
Evaluations, and commendations and reprimands over the four year period of most recent active
duty for the employee. Performance Evaluations filed with the Human Resources Division as of
the date of posting of the promotional opportunity will be included in work record scoring. For
purposes of this subsection, a"full year"is defined as an annual evaluation period in which the
employee is absent from duty and/or on restricted duty status no more than 25% of the total time.
Performance Evaluations: 15 points(maximum)
Commendations/Reprimands: 5 points(maximum)
Total Work Record Score: 20 points(maximum)
Performance Evaluation Scoring shall be as follows. Scoring will be based on the
following five (5) performance criteria found on the Civil Service Commission Annual
Performance Evaluation Report:
1. Initiative
2. Quality of Work
3. Job Knowledge
4. Customer Focus
5. Judgment&Decisions
The employee's performance ratings on each of the above 5 criteria will be assigned values
in whole numbers from 1 to 15 for each of his or her three(3)most-recent performance evaluations,
with 1 for the lowest"Unsatisfactory"rating on each criterion to 15 for the highest"Outstanding"
rating. The average value on each criterion will then be calculated using the employee's three
most-recent evaluations. All average values will be rounded to the nearest whole number, where
fractions of .50 or more will be rounded up. The rounded three-year average value for each
criterion will then be assigned"points"as follows:no(0)points for an average value of 1 to 9,one
(1) point for an average value of 10 to 11, and three (3) points for an average value of 12 to 15.
The points earned for the 5 criteria will then be totaled, and this sum, ranging from a minimum of
zero (0) to a maximum of fifteen (15) points, shall constitute the employee's Performance
Evaluation score.
Commendations/Reprimands Scoring: Shift reprimands and commendations shall only be
considered if issued within the twelve(12)month period ending thirty(30)calendar days prior to
the posting of the position. All other commendations and reprimands shall only be considered if
issued within the forty-eight (48)month period ending thirty(30) days prior to the posting of the
position.
Shift/Division Commendation 0.5 points
Department Letter of Commendation 1.0 points
13
