Exhibit A <br /> City of Decatur <br /> Northeast Watermain Extension Construction Services Project <br /> Scope/Fee/Agreement Assumptions <br /> 1. This Agreement and any legal actions concerning its validity, interpretation, and performance <br /> shall be governed by the laws of the State of Illinois. <br /> 2. Local permits for this project will be obtained by the City with information provided by Clark <br /> Dietz, Inc. All permit fees will be paid by the City. <br /> 3. State permits for this project will be obtained by the City with information provided by Clark <br /> Dietz, Inc. All permit fees will be paid by the City. <br /> 4. This Agreement does not include obtaining or processing Federal permits. <br /> 5. This Agreement does not include field staking for right-of-way or easements. Clark Dietz, Inc. <br /> does not perform construction staking. <br /> 6. This Agreement does not include the preparation of assessment roles or schedules. <br /> 7. Assumes 1 employee 12 hours per week, for six months from June 1,2026 to December 1, 2026. <br /> Additional hours based upon tasks as needed for additional services such as submittal reviews and <br /> additional critical field activities requiring additional oversight. Services may be extended per <br /> City direction. <br /> 8. Mileage to/from the City of Decatur will not be charged, but standard daily vehicle rate of <br /> $25/day will be invoiced as it relates to the scope of services. <br /> 9. Assumes 10 Contractor submittals will need to be reviewed by Clark Dietz, Inc. staff. <br /> 10. Technical oversight and guidance will be given during site visits. It is Clark Dietz, Inc. <br /> understanding that City of Decatur will be providing full-time observation,and field <br /> documentation. <br /> A-2 <br />
