For this project,bids were received from one contractor <br /> (Burdick Plumbing&Heating Co.,Inc.). <br /> CDM Smith has reviewed the bid and associated documents submitted by this bidder and has the following <br /> review comments: <br /> • The bid was received by the City by the appropriate deadline. <br /> • The bidder acknowledged receipt of Addendum 1 issued on March 6,2026. <br /> • The bid contained the required 10%bid security. <br /> • The bid contained the Subcontractor List. <br /> • The bid contained completed forms related to the City's Minority Business Enterprise(MBE) <br /> program. <br /> • The bid contained completed forms related to IEPA requirements summarized below: <br /> o IEPA Disadvantaged Business Enterprise(DBE) Program Form#1. <br /> o IEPA DBE Program Forms#3 and#4 were not completed. An MBE subcontractor was <br /> submitted.It is CDM Smith's understanding that the MBE is qualified by the City to meet the <br /> City's MBE requirements through the Decatur ePrismSoft System.However their self- <br /> certification certificate appears to have expired November 3,2025.The MBE subcontractor <br /> is Mareco Enterprises providing seeding,excavation and trucking for a participation rate of <br /> 4%of the total contract value. <br /> o Non-Collusion Affidavit. <br /> o Bidder Certification Regarding Compliance with Article 33E-11"Criminal Code of 2012". <br /> o Certification Regarding Debarment,Suspension,and Other Responsibility Matters. <br /> o U.S.EPA Certification of Nonsegregated Facilities. <br /> o Notice to Labor Unions or Other Organizations of Workers Nondiscrimination in <br /> Employment. <br /> o Bidder Certification Regarding Use of Iron,Steel,Manufactured Products,and Construction <br /> Materials Produced in the United States(Build America,Buy America Act). <br /> o Proof of Notice to Disadvantaged Businesses. <br /> Based upon our review,CDM Smith has the following recommendations: <br /> 1. We recommend that the City's legal counsel review the lowest bid(Burdick Plumbing&Heating Co., <br /> Inc.)to verify no irregularities are present from a legal standpoint. <br /> 2. We recommend that the City review the low bid submission and associated documentation to verify <br /> no irregularities are present. <br /> 3. Assuming favorable results from the legal and staff reviews recommended above,CDM Smith <br /> recommends that the City award the above listed construction contract to Burdick Plumbing& <br /> 0 <br /> WATER+ENVIRONMENT+TRANSPORTATION+ENERGY+FACIUTIES <br />
