McCrady, City Manager <br /> Keith D.Alexander, Director of Water Management <br /> SUBJECT: Lake Decatur Dam Repair Project—Phase II <br /> Inspection Engineering Services Agreement, Change Order#1 <br /> City Project 2012-25 <br /> SUMMARY RECOMMENDATION: <br /> It is recommended that the City Council approve a resolution authorizing the Mayor to execute <br /> Change Order#1 to the Inspection Engineering Services Agreement between the City of Decatur and <br /> Hanson Professional Services to provide construction inspection services for Phase II of the Lake <br /> Decatur Dam Repair Project. The resolution is for an additional $11,700 to perform the necessary <br /> construction inspection services. This is added to the original agreement amount of$143,306 for a <br /> final authorized expenditure of$155,006. <br /> PRIOR COUNCIL ACTION: <br /> See Appendix A for prior council actions. <br /> BACKGROUND: <br /> Project Status <br /> The Lake Decatur Dam Repair- Phase II project is complete with only punch list items remaining. <br /> The project installed a large amount of rip rap downstream of the dam and concrete patching <br /> occurred on the downstream face of the dam. Fiber optic conduit, a reserve water main and sludge <br /> line crossings of the Sangamon River were installed downstream of the dam. <br /> Engineering Agreement Change Order#1 <br /> Hanson Professional Services provided inspection assistance and construction approval services for <br /> the dam repair project. The time spent on the project exceeded their original estimate. Hanson has <br /> requested an additional$11,700 be allocated for the project to provide for the additional hours <br /> expended on the project. Hanson's letter of request contains further detail and is attached to the <br /> proposed Change Order included herewith. <br /> SCHEDULE: <br /> The Lake Decatur Dam Repairs Phase II Project is substantially completed and the project is <br /> currently in the closeout phase with minor punch items remaining. It is expected that final closeout <br /> of the project will be completed by the end of the year. <br /> Page 11 <br />
