APPENDIX A PRIOR COUNCIL ACTIONS REGARDING THE LAKE DECATUR DAM PROJECT August 16, 2010—Council approved Resolution R2010-159 authorizing Change Orders No. 2,No. 3, and No. 4 covering repairs to the north bascule gate. Council authorized additional funding in the amount of$280,233.22 which resulted in a total authorized project amount of$3,128,137.77. May 2, 2011 —Council approved Resolution 82011-57 authorizing amendment No. 4 with Hanson for construction engineering services to assist with the inspection of the ongoing repairs to the Lake Decatur Dam for a fee not to exceed $104,900. The amendment authorizes an increase in the total engineering services fee from$296,582 to $401,482. August 15, 2011 —Council approved Resolution R2011-131 authorizing additional funding, in the amount of$263,885.57,be added to the contract for the Lake Decatur Dam Repairs, City Project 2009- 02, for a total project cost of$3,392,023.34. February 6, 2012—Council approved Resolution R2012-05 authorizing an agreement with Hanson Professional Services, Inc. to prepare an Asian Carp protection plan for Lake Decatur at a fee not to exceed $63,700. June 18, 2012—Council approved Resolution 82012-130 authorizing an agreement with Hanson Professional Services, Inc., to obtain the necessary permits and design repairs to the tail water dam, in the amount of$82,300. This review recommended the removal of the tail water dam. August 20, 2012—Hanson Professional Services, Inc. presented the results of their Asian Carp protection plan, detailing the study's conclusions and recommendation to install a barrier on top of the dam. November 5, 2012—Council approved Resolution 82012-185 authorizing an agreement with Hanson Professional Services, Inc. to design the second phase of improvements to the Lake Decatur Dam for a fee not to exceed $89,703. July 1, 2013—Council tabled discussion regarding the bids for the Lake Decatur Dam Project - Phase Il. July 15, 2013—Council approved Resolution 82013-74 accepting the bid and awarding the contract for the Lake Decatur Dam Repairs Project—Phase II, City Project 2012-25, to Engineering & Construction Innovations, Inc., as the lowest responsible bidder in the amount of$4,159,573. August 19, 2013—Council approved Resolution R2013-86 authorizing an Agreement with Hanson Professional Services, Inc. in the amount of$143,306 to provide construction engineering services for the Lake Decatur Dam Repairs Project—Phase II, City Project 2012-25.
