Pre-Bid Meeting Minutes
' North Street Sidewalk Vault
City Project 2014-33
' The North Street Sidewalk Vault pre-bid meeting was held on Thursday,June 12,2014 at 10:00 A.M. at
Huston Patterson in Decatur, IL.
' Those in attendance were:
' Name Organization Telephone
Alex Benz Chastain&Associates 217-422-8544
Kevin Myers Chastain&Associates 217-422-8544
' Matt Newell City of Decatur 217-424-2747
Dave Horn City of Decatur 217-424-2747
Don Cisco City of Decatur 217-424-2747
' Bob Whitten Huston Patterson 217-853-1600
Richard Hathaway Huston Patterson 217-520-6154
' Mike Holst Otto Baum 217-358-1108
Thomas Kenney Christy-Foltz 217-428-8601
Brett Seward Evan-Mason, Inc 217-522-3396
1 The following items were discussed:
' 1. All debris must be removed from the vault prior to filling. Debris cannot be used as fill.
2. The garage door and any items attached to the window or frame shall be removed as part of the
pay item VAULT CONSTRUCTION.
' 3. The drain pipe for the planter box shall be capped off below the elevation of the bottom of the
sidewalk.
4. The existing telephone and water lines shall be protected during construction.
' 5. The existing concrete encased beams shall be removed. It was discussed that these may be able
to remain in place, but since they will have to be detached to remove the top 1 foot of the
existing vault wall, they must be removed. Removal of the beams will also allow for a more
efficient compaction of the fill material and eliminate notching the wall around the beams.
6. A freight elevator can be made available if necessary, but the contractor is encouraged to use
' the vault opening to transport any material into the basement or vault.
7. The contractor will be required to obtain any permit necessary to perform the vault
construction.
' 8. The adjacent parking lane can be blocked off by the contractor during construction using
appropriate IDOT standards for traffic control which will not be paid for separately but shall be
' considered as included in the lump sum unit price bid for Traffic Control and Protection,
(Special).
9. The fence around the vault area shall be removed and disposed of by the contractor.
t
