Chastain & Associates <br /> 2-13-2014 <br /> Summary <br /> MMA will provide construction phase services for the project known as the Downtown Decatur <br /> Streetscape, Franklin Street Corridor to project engineers Homer L. Chastain&Associates (Chastain & <br /> Associates). <br /> MMA is prequalified by IDOT to use the established fee schedule attached. <br /> Project Area <br /> The Project Area includes the street rights-of-way along Franklin Street,from Eldorado Street south <br /> to Wood Street, a distance of five blocks. The project area includes short distances to the east and west of <br /> Franklin along intersecting streets as "tie-ins" with the corridor improvements. <br /> Scope of Services <br /> MMA provide the following services: <br /> 1. Answer questions that arise during construction. <br /> 2. Assist with the review and approval of submittals and materials provided for the project. <br /> 3. Assist in preparing change orders that may arise due to unforeseen site conditions. <br /> 4. Assist in the preparation of final as-built drawings to document the final condition <br /> of the project. <br /> 5. Other construction engineering assistance as requested by the City including on-site <br /> observations and reports, and inspections and punch lists. <br /> 5 <br />
