<br /> It is a pleasure to advise you that your application for a wastewater treatm nt works loan, under
<br /> the provisions of the Environmental Protection Act, has been approve in the amount of
<br /> $8,588,792.50 at a 1.93 percent simple annual interest rate.
<br /> The Loan Agreement is enclosed in triplicate. After the Agreement is reviewed, the original and
<br /> one copy should be signed by the appropriate official and returned to the I astructure Financial
<br /> Assistance Section, Bureau of Water, P.O. Box 19276, Springfield, IL 62794-9276 as soon as it
<br /> is signed, but no later than September 15, 2013. The second copy is for yo r records. Receipt of
<br /> a written refusal or failure to return the properly executed documents within the specified time
<br /> will subject the Loan Agreement to cancellation consideration.
<br /> Your attention is invited to the Standard Conditions of the Loan Agreeme t which outlines the
<br /> requirements and terms of your loan. The schedule is based on your a ding contracts and
<br /> initiating project construction promptly. Your compliance with the loan onditions and terms
<br /> will facilitate our processing and your receipt of loan disbursements. In addition, by acceptance
<br /> of this loan agreement, the loan recipient agrees to comply with any future reporting and/or
<br /> accountability requirements that may be associated with the State Revolv' g Fund programs in
<br /> Illinois.
<br /> In accordance with the Procedures For Issuing Loans From The Water Pc llution Control Loan
<br /> Program Title 35111. Adm. Code Part 365 (Loan Rules) concerning project nitiation, we endorse
<br /> the award of the construction contract for the scope of work identified int e Project Description
<br /> to SAK Construction LLC in the allowable amount of $4,177,792.75 fo Division A, and to
<br /> Insituform Technologies USA, LLC in the allowable amounts of$1,348,734.25 and
<br /> $1,913,257.50 for Divisions B and C, respectively.
