Exhibit A <br /> SCOPE OF WORK <br /> (Describe the scope of work by breaking down the project into major work elements or phases. <br /> Further divide the major work elements or phases into tasks, identify proposed staff positions to <br /> accomplish the tasks, and show direct salary cost. Estimate the staff hours to accomplish the <br /> task, and calculate the direct cost. A sum of staff hours by position and direct salary cost should <br /> be made for each of the major work elements or phases. The total estimated labor cost is the sum <br /> of all the elements or phases multiplied by the approved project multiplier. The total estimated <br /> engineering cost is the total labor cost plus estimated reimbursable expenses. Alternately, <br /> replace this Exhibit with a spread sheet defining scope of work. Mark the spreadsheet Exhibit <br /> A.) Number pages "A-1", "A-2"and so on. <br /> Project Description: <br /> Provide professional services related to construction related activities for the Lake Decatur Dam <br /> Repairs—Phase 2. <br /> Services: <br /> The Scope of Services to be provided is limited to the following. These services shall modify the <br /> Basic Services, Section II.0—Construction Inspection Phase of the Agreement. <br /> 1. Pre-Construction Meeting: Attend on-site pre-construction meeting to discuss project scope <br /> with the selected contractor. <br /> 2. Construction Observation: <br /> a. Hanson will visit the site periodically,as directed by the City's on-site representative to <br /> assist with observation of the work for compliance with the contract documents. <br /> b. Assume Hanson's on-site representative(s)will visit the site an average of three(3)times <br /> per week for a duration of 60 weeks,plus expenses for travel to the site. <br /> c. Assume five(5)on-site visits by Hanson design representatives to observe the work. <br /> 3. Contractor Submittals: Review submittals prepared by the Contractor for items specified to <br /> be included in the work for conformance with the project specifications. Fifty nine(59) <br /> separate submittals are specified:assume 3 hours per submittal to review and process. All <br /> submittals are assumed to be electronic via the project website. <br /> 4. Contractor Pay Requests: Review requests for payment by the Contractor for agreement with <br /> observed progress of the work. Assume fourteen(14)monthly pay applications.All <br /> submittals are assumed to be electronic via the project website. <br /> 5. Respond to Contractor's Request for Information(RFI). Assume twenty(20)RFI received <br /> by Hanson's designers to clarify project construction documents. All submittals are assumed <br /> to be electronic via the project website. <br /> 6. Review Contractor Value Engineering proposal and process Change Orders. Assume two <br /> (2) <br /> A-1 <br />
