Anderson Plaza, Decatur IL 62523 (address) until 10:00 o'clock A M., August 7, 2013 Proposals will be opened and read publicly (d'ate) ' at 10:00 o'clock A M., August 7, 2013 at the office of City Engineer 1 Gary K Anderson Plaza, Decatur, IL 62523 (date) (address) ' Description of Work Name Street Restoration Length 10573.00 feet ( 2.00 miles) ' Location Various City Streets Proposed Improvement Resurface/Reconstruct various City streets with hot-mix asphalt surface and cement ' stabalized base as specified. Remove, replace, adjust manholes&valve boxes. Class B &D patching. Bidders Instructions t1. Plans and proposal forms will be available in the office of Decatur Blue Print, Inc., 230 W Wood Street Decatur, IL 62523 for the cost of$75.00. IDOT Certificate of Eligibility Required. ' 2. If prequalification is required , the 2 low bidders must file within 24 hours after the letting an"Affidavit of Availability" (Form BC 57), in triplicate, showing all uncompleted contracts awarded to them and all low bids pending award for Federal, State, County, Municipal and private work. One copy shall be filed with the Awarding Authority and 2 copies with the IDOT District Office. ' 3. All proposals must be accompanied by a proposal guaranty as provided in BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals. ' 4. The Awarding Authority reserves the right to waive technicalities and to reject any or all proposals as provided in BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals. 5. Bidders need not return the entire contract proposal when bids are submitted unless otherwise required. Portions of the proposal that must be returned include the following: a.
