APPENDIX A <br /> August 16, 2010—Council approved Resolution R2010-159 authorizing Change Orders No. 2,No. <br /> 3, and No. 4 covering repairs to the north bascule gate. Council authorized additional funding in the <br /> amount of$280,233.22 which resulted in a total authorized project amount of$3,128,137.77. <br /> May 2, 2011—Council approved Resolution R2011-57 authorizing amendment No. 4 with Hanson <br /> for construction engineering services to assist with the inspection of the ongoing repairs to the Lake <br /> Decatur Dam for a fee not to exceed $104,900. The amendment authorizes an increase in the total <br /> engineering services fee from$296,582 to $401,482. <br /> August 15, 2011 —Council approved Resolution R2011-131 authorizing additional funding, in the <br /> amount of$263,885.57, be added to the contract for the Lake Decatur Dam Repairs, City Project <br /> 2009-02, for a total project cost of$3,392,023.34. <br /> February 6, 2012—Council approved Resolution R2012-05 authorizing an agreement with Hanson <br /> Professional Services, Inc. to prepare an Asian Carp protection plan for Lake Decatur at a fee not to <br /> exceed $63,700. <br /> June 18, 2012—Council approved Resolution R2012-130 authorizing an agreement with Hanson <br /> Professional Services, Inc., to obtain the necessary permits and design repairs to the tail water dam, <br /> in the amount of$82,300. This review recommended the removal of the tail water dam. <br /> August 20, 2012—Hanson Professional Services, Inc. presented the results of their Asian Carp <br /> protection plan, detailing the study's conclusions and recommendation to install a barrier on top of <br /> the dam. <br /> November 5, 2012—Council approved Resolution R2012-185 authorizing an agreement with <br /> Hanson Professional Services, Inc. to design the second phase of improvements to the Lake Decatur <br /> Dam for a fee not to exceed $89,703. <br /> June 1, 2013—Council tabled discussion regarding the bids for the Lake Decatur Dam Project - <br /> Phase II. <br /> Page 15 <br />
