1. The City shall have the responsibility for establishing, and the City reserves the right <br /> to amend, the guidelines and requirements of Operation Uplift, including the service area <br /> and the hours of operation, at any time. <br /> 2. The City shall communicate the guidelines and requirements of Operation Uplift, <br /> including the service area and the hours of operation, and any amendments thereto, to the <br /> Livery Company in a timely manner. <br /> 3. The City shall register applicants for Operation Uplift, determine their eligibility, and <br /> furnish proper photo-identification to all eligible, certified participants. <br /> 4. The City shall periodically submit updated lists of eligible Operation Uplift participants <br /> to the Livery Company. <br /> 5. The City shall provide adequate supplies of Livery Ride Forms to individual <br /> participants, the Livery Company, social agencies, and other interested parties. <br /> 6. The City shall promote Operation Uplift as the City deems necessary,by whatever <br /> means the City chooses. <br /> FARES AND SUBSIDIES <br /> The City shall reimburse the Livery Company for the amount of fare subsidies and non-cash fare <br /> payments based on the fares and fare subsidy amounts recorded on the Livery Ride Forms. Fares <br /> and fare subsidies under Operation Uplift shall be governed by the following: <br /> 1. The fares charged by the Livery Company to Operation Uplift participants during the <br /> first and second years of this Agreement are shown in the following table: <br /> YEARI. YEAR 2 <br /> First Passenger, trips within Decatur $11.00 $13.00 <br /> First Passenger, trips to Forsyth $13.00 $15.00 <br /> First Passenger, trips to Harristown $13.00 $15.00 <br /> First Passenger, trips to Long Creek $14.00 $16.00 <br /> Surcharge for trips made by a rider in a wheelchair* <br /> using a wheelchair accessible van $2.50 $2.50 <br /> (*including electric scooters and similar mobility aids) <br /> Each additional passenger, 5 years of age and older $3.00 $3.00 <br /> Each additional passenger, less than 5 years of age FREE FREE <br /> Personal Care Attendant (PCA) FREE FREE <br /> 2. If the City chooses to extend this Agreement for additional one (1) year terms (option <br /> years), then the fares to be charged in each option year shall be agreed upon by the City <br /> and the Livery Company before the commencement of each option year. <br /> 3. If one or more Operation Uplift riders are traveling from a single origin to a single <br /> destination, then the total fare shall be the charge for one "First Passenger"trip plus charges <br /> for the other riders and escorts at the appropriate "Each additional passenger"rates. <br /> - 7 - <br />
