Exhibit A <br /> LAKE SHORE DRIVE SEWER REHABILITATION <br /> DESIGN ENGINEERING <br /> SCOPE OF WORK <br /> The project is to be known as LAKE SHORE DRIVE SEWER REHABILITATION. The PROJECT <br /> shall consist of the design of the rehabilitation improvements to the City owned Lake Shore Drive <br /> (Southeast Interceptor)including the Lake Shore Drive—East Trunk, Lake Shore Drive—West Trunk, <br /> Lake Shore Drive— 16th Street Branch and Lake Shore Drive—MLK Branch sewers as outlined in the <br /> City of Decatur's Critical Sewer Rehabilitation Facilities Plan Amendment dated February 2012 by <br /> Bainbridge, Gee,Milanski&Associates. The project will be bid as a single project with multiple <br /> divisions of work. This project is expected to be funded with an Illinois EPA SRF loan. <br /> THEREFORE,In Consideration of these premises and the mutual covenants of the above described <br /> PROJECT: <br /> THE ENGINEER AGREES, <br /> 1. Proiect Engineering Services -To perform or be responsible for the performance of the <br /> following professional engineering services for the OWNER in connection with the design of the <br /> proposed projects hereinbefore described: <br /> a. Make such detailed surveys as are necessary for the preparation of detailed construction <br /> plans. <br /> b. Prepare detailed rehabilitation construction drawings and specifications in such a manner <br /> as to permit the OWNER to obtain construction bid prices for the above described <br /> PROJECT. <br /> C. Prepare proposal forms,notice to bidders, and the Contract Documents. <br /> d. Prepare estimate of project construction cost based on final plans and documents. <br /> e. Furnish copies of the Contract Documents consisting of Construction Agreement Forms, <br /> General Conditions, Special Conditions,Detailed Construction Plans and Specifications <br /> for review by the Owner, and for submission to permitting authorities. <br /> f. Incorporate OWNER's review comments into the final set of contract documents. <br /> g. Assist the OWNER with necessary permit applications to all governmental and other <br /> authorities having jurisdiction to approve portions of the PROJECT such as,but not <br /> limited to,the Illinois Environmental Protection Agency, City of Decatur,Division of <br /> Water Resources,and U.S.Army Corps of Engineers. <br /> h. Provide the OWNER with assistance in obtaining project financing utilizing a loan from <br /> the Illinois Water Pollution Control Revolving Fund. <br /> i. Project Time of Performance-To complete the engineering design services within 280 <br /> calendar days from the receipt of a"Notice to Proceed" from the OWNER. <br /> 2. Project Required Provisions <br /> a. The ENGINEER agrees to take affirmative steps to assure that disadvantaged business <br /> enterprises are utilized when possible as sources of supplies, equipment, construction, <br /> A-1 <br />
