EXHIBIT A <br /> WATER RESOURCES DIVISION <br /> October 28, 2011 <br /> TO: Mr. Steve Leach <br /> City of Decatur <br /> RE: Cisco Well No. 2 Pump Repair <br /> Dear Mr. Leach: <br /> The Byron Jackson oil-lubricated pump has been pulled for repairs and brought into our <br /> yard for cleaning and inspection. We wish to submit this letter as our report on the <br /> inspection results and provide repair cost estimates. <br /> As you know, the pump was pulled due to locked up condition. Once the motor and first <br /> pipe and tube section had been removed the shaft and pump turned free down hole. <br /> For a pump that has not been repaired reportedly since the initial installation in the <br /> 1950s, it is in excellent condition. A breakdown of the various components and their <br /> conditions are: <br /> 40 HP VHS Electric Motor: This item was left on site for your repair at your local repair <br /> facility. We would like to know what was found as at issue since the system turned free <br /> once the motor was removed and our first pipe and tube sections were removed. <br /> 12" Byron Jackson Discharge Head: The heavy duty cast iron head is suitable for <br /> reuse. We would check over the tension assembly and refurbish as necessary. We <br /> would also recondition the head by sandblasting, priming, and coating inside and out. <br /> 12" Column Pipe: The 12" pipe was in very good condition, normally requiring power <br /> washing to clean off the mineral deposits for inspection. There was significant pitting on <br /> the bottom end of the 5 foot piece of pipe off the bowl assembly, and the top 5 foot <br /> piece of pipe into the discharge head had to be cut. Therefore, 2 - 12" x 5 foot nominal <br /> length pipe need to be furnished, one coming with a new 12" coupling. There is one 12" <br /> coupling that needs to be replaced also. <br /> 3 Inch Oil Tubing: Due to the mixture of water and oil on the inside of the oil tubing, it <br /> all had to be disassembled, cleaned out, and inspected. There is one short piece of oil <br /> tubing that needs to be replaced that is located just off the bowl assembly. The inside <br /> of the oil tubing will be cleaned again before reassembly. <br /> 1.50 Inch Line Shafts and Shaft Couplings: There is some minimal wear on the line <br /> shafts at the bronze bearing points, but all shafts are suitable for reuse.
