On October 6, 2011 the City Plan Commission voted 9-0 to approve the rezoning from R-5 <br /> Special Use Permit and R-5 Residential Development Plan (R-5 RDP) to PD Planned <br /> Development. The petition was approved by City Council by a 7-0 vote on October 17, 2011. <br /> 2. The petitioner proposed to rezone property from R-5 Special Use Permit and R-5 Residential <br /> Development Plan to PD Planned Development to allow for the construction of a 20 unit memory <br /> care facility. The proposed facility would complement the already existing nursing home to the <br /> west, the independent living facility to the east and the assisted living facility immediately to the <br /> south. <br /> 3. The proposed development would be a single story structure that was designed to complement the <br /> existing buildings and facilities. <br /> PROJECT DESCRIPTION <br /> 1. The petitioner proposes to amend the PD Planned Development Plan to still allow for the <br /> construction of a 20 unit memory care facility. The development is still proposed to be a single <br /> story structure. <br /> 2. When the developer was in the process of obtaining the necessary State of Illinois approvals it <br /> was a requirement of the State of Illinois Department of Public Health that the proposed new <br />
