Chapter 41 <br /> An executed Indemnity Agreement in a form approved by <br /> the City. <br /> (7) Certificates of insurance as per the requirements established herein. <br /> () Any other objeainformation related to the requirements of this chapter that the <br /> Public Works Director may require. <br /> D. REVIEW OF APPLICATION. <br /> (1) The Public Works Director or designee shall review the application for an <br /> outdoor cafe permit and determine whether the permit shall be ,,.eatea%%thin fou_/A\feet,.issued. <br /> The decision of the Public Works Director shall be a final administrative decision. <br /> (2) The applicant shall supply all of the information required on or by the applica- <br /> tion, and any interseetion,as defined in the 111ineis Vehiele Code, and s ra table igdditional information as <br /> reasonably requested by the Public Works Director as a condition precedent to the issuance of a <br /> permit. <br /> E.
