Amendment NO. 4 tO <br /> Agreement Co8Lo198 <br /> Whereas,the City of Decatur, subsequently referred to as "Client,"and Hanson Professional Services Inc., <br /> subsequently referred to as"Hanson,"have previously entered into a Professional Services Agreement dated <br /> December 29, 2008 in connection with the Lake Decatur Dam Repairs subsequently referred to as"Project,"and <br /> Whereas,the Client has ordered certain changes to the services being provided by Hanson for the Project. <br /> Now,therefore, this Amendment to the Agreement(Amendment No. 4)is made this 28th day of March,2011 to <br /> revise the Scope of Services and Cost of Services as provided herein. All other terms and conditions of the <br /> Agreement remain unchanged. <br /> The Scope of Services is modified as follows: <br /> 1. 2011 Construction Services: <br /> Provide the following construction services during 2011 for the completion of the Lake Decatur <br /> Dam Repair Work: <br /> a. Construction Observation—Assume 6 month duration; average 6 hours per day, 3 days <br /> per week. Includes on-site observation and coordination with the Contractor. <br /> b. Review Contractor Submittals -Assume 4 submittals for sluice gate seals and machinery. <br /> C. Review Contractor Pay Requests-Assume 6 applications for payment. <br /> d. Respond to Contractor Request for Information(RFI)—Assume 20 RFI. <br /> e. Administer Contract Change Order Requests—Assume 8 Change Orders. It is <br /> anticipated that change orders, similar to those in 2010 for the north bascule gate seals, <br /> will be required for the south bascule gate.
