� EXHIBIT A <br /> SCOPE OF WORI� <br /> The project shall consist of the preparation of a Facilities Plan foi-the 7t�'Ward, Lalce Shore, <br /> McKinley and Union Street Sewers Rehabilitation. The rehabilitation of these sewers is intended <br /> to be financed with a loan fi•om the Illinois Envirorunental Protection Agency Water Pollution <br /> Control State Revolving Fund (SRF) program. One of the documents required to complete an <br /> application for a SRF loan, is an approved Facilities Plan. <br /> The scope of this project is to prepare a Facilities Plan for this project in accordance with the <br /> requirements of Title 35, Chapter II, Part 365 "Procedures For Issuing Loans From the Water <br /> Pollution Control Loan Program". Subsection 365.520 lists the requirements for a facilities <br /> plan. <br /> This Facilities Plan will address four sewers; Lake Shore Drive Sewer(Southeast Interceptor), <br /> McKinley Avenue Sewer, 7�'Ward Sewer, and the Union Street Sewer. These sewers are <br /> defined in the 2009 Sanitary Sewer Master Plan prepared by Bainbridge, Gee, Milanslci & <br /> Associates. The plan will identify each sewer, describe its current conditions, summarize the <br /> defects, and list alternatives for making necessary repairs. Alternatives will be compared on a <br /> life cycle cost basis. One alternative will be recommended and its environmental impacts will be <br /> discussed. The impact of the project on the City's user fees will be calculated and presented. <br /> An implementation plan will be developed with dates for action by the City. The report will be <br /> sent to the necessary state and federal agencies for approval. Once the IEPA issues theu• <br /> Preliminary Environmental Impacts Determination(PEID), a public hearing will be conducted <br /> with the City. The City will be responsible for advertising the public hearing and furnishing a <br /> place for the hearing. BGM will conduct the hearing. The rehabilitation of each sewer will be <br /> separate from the other sewers. The plan will be written in such a form as to allow the City to <br /> apply for loan funding for each sewer rehabilitation separately. <br /> The basic information on the condition of the sewers will be taken from the 2009 Sanitary Sewer <br /> Master. No additional sewer or manhole inspections will be conducted for this report. No new <br /> field surveys, flow monitoring or sampling will conducted. No archeological reconnaissance <br /> surveys, wetland delineations,material testing, ar hydraulic studies are included in this study. <br /> These investigations are not anticipated to be required for IEPA to approve this report and are <br /> therefore not included in the scope of work. <br /> Representatives of IEPA are saying to expect the Facilities Plan review to take a minimuin of 12 <br /> to 18 inonths. This review time is a function of the number of plans submitted for review and the <br /> number of reviewers available. <br /> A-1 <br />
