�• <br /> ATTACHMENT A <br /> WORK ORDER NO 4 <br /> AGREEMENT FOR SERVICES DATED FEBRUARY 6, 2007 <br /> RE: City of Decatur <br /> Eldorado Streetscape—Church Street to CNRR Tracks <br /> Work Order No. 1 dated February 6, 2007 provided for Phase II design services. <br /> Work Order No. 2 dated March 2, 2009,provided for Phase IlI construction services and <br /> Phase II design services in addition to those provided for in Work Order No 1 <br /> Work Order No. 3 dated May 21, 2009 provided for Phase II design of additional lighting <br /> from Martin Luther King (MLK) Drive to the Canadian National Rail Road(CNRR) <br /> Tracks. This lighting was added to the project after award of the construction contract. <br /> A. Work Order No 4 - Scope of Work: <br /> 1. Provide additional Phase III services since the original estimated fee in Work Order No. 2 <br /> has been expended and construction is not completed. Construction is anticipated to be <br /> complete in July 2010. : <br /> a. Answer questions during construction of the project. <br /> b. Review shop drawings and product samples. <br /> c. Render opinions on the Contractor's compliance with the contract documents. <br /> d. Provide design to resolve conflicts with ui�iown conditions. <br /> e. Attend final punch-list inspection. <br /> f. Other services as requested by the Client. <br /> 2. Provide additional Phase II services to submit information and calculations to <br /> IDOT in order to resolve issues with the lighting design being proposed by others for the <br /> Franklin Street overpass project, and, if necessary. provide Phase II design services to , <br /> revise the lighting design at the Franklin Street/Eldorado Street intersection II <br /> 3. Provide additional Phase II design services necessary to complete the designs and plans I <br /> provided for under Work Order No. 1 and Work Order No. 2. Unforseen services were <br /> required mostly related to final lighting designs and revisions to plans resulting from <br /> final easement negotiations. <br /> B. Schedule <br /> 1. The services under A.1 and A2 will be performed during the construction contract <br /> time frame.
