Professional Services Agreement <br /> C09L0143 <br /> Attachment A - Scope of Services <br /> Agreement Date: September 10 2009 <br /> Project: 2009-2011 Annual Dam Inspection Lake Decatur Dam Dred�g Sediment Storage Facility, Decatur, <br /> Illinois <br /> Project Description: <br /> The project includes providing the annual dam inspection for the Lake Decatur Dam in Decatur,Illinois and far the <br /> Dredging Sediment Storage Facility in Oakley Township,Illinois for a period of three years starting in 2009. <br /> Services: <br /> The Scope of Services to be provided is limited to the following: <br /> A registered professional engineer will perform the annual dam inspection for the Lake Decatur Dam in Decatur, <br /> Illinois and for the Dredging Sediment Storage Facility in Oakley Township,Illinois.Hanson will coordinate the <br /> performance of the inspections with the City of Decatur and with the Illinois Department of Natural Resources,Office <br /> of Water Resources.The City of Decatur will provide Hanson with copies of the most recent annual inspection <br /> reports for each of these facilities as references and as baseline information in order to more efficiently perform the <br /> requested annual inspections. <br /> The reporting of these inspections will follow the format presented in the Illinois Department of Natural Resources <br /> publication,"Guidelines and Forms for Inspection of Illinois Dams,November, 1999."Copies of these annual <br /> inspection reports will be provided to the City of Decatur for submittal to the Illinois Department of Natural <br /> Resources, Office of Water Resources. <br /> PSA R�Printed on recycled paper using agri-based inks � <br /> _ _ _ <br />
