Attachment A <br /> Work Order No. l <br /> RE: City of Decatur <br /> Garfield Avenue over IL 121 <br /> S/N 058-6001 <br /> Special Feature Pin&Link Inspection <br /> A. Scope of Work <br /> The following is a summary of the work that URS will perform: <br /> l. Review the Inspection Appraisal and Inventory Forms S-104&S-105 which URS will download <br /> from the Illinois Department of Transportation web site for the following structure: <br /> 058-6001 Gaifield Avenue Illinois Route 121 <br /> 2. Perform a hands-on inspection, utilizing visual and ultrasonic testing equipment in sufficient detail <br /> to allow completion of the required IDOT bridge inspection reports (BBS-SFI-1 &BBS 2760), and <br /> revision of the S-104 if needed. <br /> 3. Provide at least four (4) color photographs and one (1) 8 '/z" x 11" drawing of the special feature <br /> detail in order to accurately document the condition of this special feature detail. <br /> 4. All inspections will be completed in accordance with the procedures and terminology outlined in <br /> Sections 33 and 3.5 of the Illinois Department of Transportation (IDOT) Structural Services <br /> Manual, and all other applicable and approved procedures and methods as noted in the Federal <br /> Highway Administration (FHWA) Bridge Inspectors Reference Manual, FHWA Recording and <br /> Coding Guide for the Structure Inventory and Appraisal of the Nation's Bridges, and IDOT <br /> Structure Information and Procedure Manual. <br /> 5. The scope of this work follows the basic pass-fail criteria for pin inspection utilizing ultrasonic <br /> testing as described in Section 3.5 of the IDOT Structural Services Manual, but dces not cover the <br /> Supplemental Pin Inspection procedures as described in Section 3.5.4.2 of the same manual. <br /> 6. Coordinate the inspection activity and report filing with the City of Decatur and the Illinois <br /> Department of Transportation. <br /> B. Proiect Personnel <br /> URS maintains a full staff of qualified and experienced structural engineers who are available to <br /> perform this inspection. Mr. Jeffrey P. Bucholc, P.E., SE will serve as Project Manager for the <br /> project. Major portions of the inspections will be completed by Mr. James R. Tippett,PE. He will <br /> be assisted by Dean Y. Smith in the field inspection and documentation. They will be assisted by <br /> other members of our Structural Engineering Department as necessary to assure completion of the <br /> project on schedule. <br />
