EXHIBIT H <br /> Post Employment Health Plan (PEHP) <br /> Section 501(c)(9) <br /> Voluntary Employee Benefit Association (VEBA) <br /> Section 1. Plan. The City and the Union hereby adopt the Post Employment Health Plan <br /> (PEHP) for Collectively Bargained Public Employees of Nationwide Retirement Solutions, a <br /> Nationwide Financial Company, as amended and restated as of January 17, 1997. <br /> Section 2. Copies. Copies of the Plan Document and the Trust Agreement for the PEHP will <br /> be on file for inspection by members of the bargaining unit at each of the Fire Stations of the <br /> City of Decatur. <br /> Section 3. Election. The Union elects, and the City agrees, to apply 100% of PEHP <br /> contributions to the Qualifying Medical Care Expense subaccount of the PEHP, as defined in <br /> the Plan Document. <br /> Section 4.
