I <br /> = I <br /> i ' <br /> ORDINANCE NO. 2008-06 <br /> ORDINANCE GRANTING CONDITIONAL USE PERMIT <br /> - 461 WEST WOOD STREET - <br /> WHEREAS, on the 6th da of March, 2008, u on due noti�e, the Decatur Cit Plan <br /> Y r Y <br /> Commission held a public hearing upon the petition of Mil�e Burkham, Professional <br /> Management and Leasing, for a conditional use permit under the provisions of Section XXII.A.3 <br /> of the Zoning Ordinance to allow an ambulance service on prope y located in the City of <br /> Decatur, Illinois and legally described as follows: <br /> The East Half(E '/z) of Lot Five (5) and West 33.8 feet of Lo Four (4) in Block <br /> Two (2) of Southwestern Addition as per plat recorded in Book N, Page 440 of <br /> the Records in the Recorder's Office of Macon County, Illinois; and, <br /> WHEREAS, said Decatur City Plan Commission recommende that such conditional use <br /> permit be approved. <br /> NOW, THEREFORE, BE IT ORDAINED, BY THE CITY OUNCIL OF THE CITY <br /> OF DECATUR, ILLINOIS: <br /> Section 1. That the recommendation and report of the Decatur City Plan Commission be, <br /> and the same are hereby, received, placed on file and granted. <br /> Section 2. That a conditional use permit be, and it is hereby, granted to Mike Burkham, <br /> Professional Management and Leasing, and only to Mike Burkham, Professional Management <br /> and Leasing, to allow an ambulance service upon the premises afores id in accordance with the <br /> plat thereof hereto attached marked Exhibit A and hereby made a part ereof. <br /> Section 3. That the conditional use permit be, and the s me is hereby, expressly <br /> conditioned as follows: <br /> (a) That a detailed Site Plan, Lighting Plan and Land cape Plan shall be <br /> submitted to and approved by the Economic and Urban Develo ment Department, <br /> the Department of Engineering and Infrastructure and the Fir Department prior <br /> to the issuance of a building permit. <br />
