, ' . , ' t SANITARY DISTRICT OF DECATUR EXHIBIT `D' INSURANCE SG1 Without limiting its liability under this Contract, the Contractor shall procure and maintain, at his expense during the life of this Contract, insurance of the types and in the minimum amounts stated below: (a) Workmen's Compensation insurance in full compliance with the Workxnen's Compensation Act and Employer's Liability coverage in the amount of not less than as defined by Illinois State Statutes with the following Employers Liability limits. $100,000 Each Accident Bodily Injury by Accident $100,000 Each Employee Bodily Injury by Disease $500,000 Policy Limit Bodily Injury by Disease (b) Commercial General Liability including explosion(x), collapse (c) and underground(u) properly damage liability, and blanket contractual liability covering the liability assumed the Contractor under this agreement with limits of not less than: $1,000,000 General Aggregate Limit(other than products-completed operations) $1,000,000 Products-Completed Operations Aggregate Limit $1,000,000 Personal Injury and Advertising Injury Limits* $1,000,000 Per Occurrence Limits* $50,000 Fire/Explosion/Water Damage Limit** i $5,000 Medical Expense Limit** (any one person) ' * Subject to the applicable "aggregate limit" �I� ** Subject to the "occurrence limit" (c)Automobile Liability Insurance covering owned,non-owned and hired automobiles with limits not less than: Bodily injury and Properly Damage Liability Combined Single Limit if $1,000,000 (d) Umbrella Excess Liability coverage with a minimum per occurrence limit of$2,000,000 in excess of the above Employers Liability,Automobile Liability and Commercial General Liability policies. (e) Owner's Protective Liability policy issued to the Sanitary District of Decatur with a minimum combined single limit of$1,000,000. 1_1 All policies shall be written on an OCCURRENCE policy form unless the Contractor receives prior written approval from the Owner, to use a CLAIMS MADE policy form.
