�-' EXHIBIT D i ' � � II . � SANITARY DISTRICT OF DECATUR 1_2 The Commercial General Liability policy shall name the Sanitary District of Decatur and the (name of architect or engineer) as additional insureds. 1_3 (a)The Contractor shall also take out and maintain at his expense during the life of this Contract, Builder's Risk Insurance satisfactory to the Sanitary District of Decatur, which sha11 protect the Sanitary District of Decatur, all subcontractors, material suppliers and the Sanitary District of Decatur,the Sanitary District of Decatur's agents and the Engineer as their interest may appeaz, for loss to materials and equipment stored at the site,materials and temporary structures excluding contractor's construction equipment. 'The Builder's Risk policy shall insure against loss of perils on an "All Risk"basis including theft. Such policy sha11 be in a provisional amount equal to the total contract price, or such other amount as shall be satisfactory to the Sanitary District of Decatur, and a duplicate policy thereof sha11 be provided to the Sanitary District of Decatur through the Engineer. The Builder's Risk Insurance, as specified herein, shall be a primary coverage in relation to any ott�er policies,which the Sanitary District of Decatur may maintain on its property, including the property covered by this Contract, and the policy sha11 include a suitable endorsement to this effect. (b)The deductible sha11 not exceed$2,500 and the deductible amount shall be the responsibility of the Contractor. Coverage provided by the Contractor shall be limited and defined by the actual terms and conditions of the policy. 1_4 An insurance company or companies satisfactory to the Sanitary District of Decatur � shall write said insurance required in this section. Before commencing any work hereunder, certificates evidencing the maintenance of worker's compensation, general liability and automobile liability and umbrella liability shall be furnished to the Sanitary District of Decatur and sha11 contain substantially the following cancellation provision: "The insurance evidenced by this certificate will not be canceled or materially reduced except after 30 days from receipt by the Sanitary District of Decatur and the Engineer of written notice thereof." ' "The Worker's Compensation insurance evidenced by this certification is valid in the State of Illinois." Any subcontractor of the Contractor shall be required to procure and maintain during the life of the subcontract, insurance the same as required of the Contractor hereunder and to comply with the provisions of this article, except that the full Owner's Protective Coverage shall be provided by the General Contractor.
