. • • <br /> Classifcation of Leaks <br /> ADS maintain the following classification of leaks,which will be shown on each individual leak sheet and in the <br /> final report: <br /> Class Definition <br /> ] Leaks that are hazardous in terms of potential underground washouts, <br /> possibly resulting in surface collapse,encroachment and/or damage to <br /> nearby utilities,commercial and private properties or leaks that indicate <br /> leakage to be severe enough to warrant immediate repair. <br /> 2 Leaks that display water losses significant enough to be placed on a" <br /> regular repair schedule <br /> 3 Relatively small leaks that should be repaired as work time permits <br /> Reporting <br /> We will meet with the designated City representative and deliver any leak reports from the previous day's work. <br /> Leak sheets will be prepared with a sketch of the location,leak classification and estimated water loss.We will also <br /> prepare a progression map with symbols that indicate leaks and classification corresponding to the leak report. <br /> ADS will prepare and submit a consultant's weekly report to the City's designated representative that will include: <br /> • Miles surveyed that week <br /> • Miles surveyed to date <br /> • Totai estimated water loss � <br /> • Number of leaks and suspected locations <br /> • Hours worked that week <br /> Safety <br /> All inspection work is performed in accordance with OSHA regulations for"confined space entry".All field <br /> personnel attend yearly training and certification in Confined Space Entry,First Aid and CPR,in addition to an on- <br /> going,in-house training program.All field personnel wear appropriate company uniforms and/or identification <br /> badges,along with safety vests.Each vehicle/crew is equipped with a cellular telephone. <br /> Final Report <br /> Our final report will summarize the project and show the results of all tests and investigations.This report will <br /> include: <br /> • A comprehensive list of all leak locations and estimated quantity of leakage found <br /> • An inventory of defective system components and map errors <br /> • Sketches of the individual leak locations <br /> • Estimated benefits-to-costs resulting from completing the leak survey effort taking into account: <br /> — Estimated water loss from the leaks discovered during the survey <br /> — City's production price for water <br /> — Cost of the survey itself <br /> Schedule of Work <br /> ADS will be available to begin the survey within four weeks of notice to proceed.Based on the number of miles,it <br /> is estimated that the field portion of the project will take approximately 35 working days to complete depending on <br /> weather conditions and the number of leaks located.The technician can usually cover approximately 5 to 6 miles of <br /> pipe per day during the sounding phase,depending on weather,traffic conditions and the number of leaks identified. <br /> E�chibits ADS Environmental Services <br />
