Attachment A to Work Order#1 <br /> City of Decatur,Illinois Annual Dam Inspections <br /> September 5,2007 <br /> Scope <br /> URS Corporation will conduct visual inspections of two dams owned by the City of <br /> Decatur. The work will generally consist of review of past inspection reports and <br /> maintenance records, visual inspections, and the composition of a final inspection report <br /> for each facility. More specifically, the work will consist of the following for each <br /> facility: <br /> � Review design documents and details that the City has on record, as it is <br /> imperative that the inspection team be familiar with the design details of these <br /> particular facilities <br /> � Review past inspection reports that the City has on record. Both recent and more <br /> historical reports are of interest. <br /> � Review past maintenance records that the City may have, along with current <br /> maintenance practices <br /> � Meet with City personal who are most familiar with the facilities so as to glean <br /> any pertinent information and discuss any known issues or concerns <br /> � Visually inspect each dam and its related appurtenances from areas safely <br /> accessible by foot. No boats or other"over the water" means of access will be <br /> utilized. <br /> � Compose a written inspection report complete with any pertinent photos, figures, <br /> or drawings. Any concerns will be discussed in the respective reports along with <br /> recommended actions. <br /> � Complete internal Independent Technical Review (ITR). <br /> � Submit four(4) copies of completed report to City. The report will be sealed by a <br /> licensed professional engineer in the State of Illinois. <br /> Schedule <br /> The schedule is approximated as follows and will commence upon URS receiving a <br /> written Notice to Proceed from the City. <br /> � Review past reports, design info, etc. —3 weeks from receipt of information <br /> � Visual inspection and meeting with City personnel —2 weeks from completion of <br /> review of past information <br /> � Compose report—4 weeks from completion of visual inspection <br /> � Total duration of project estimated at 9 weeks <br /> Budget <br /> URS will complete the work described herein for a lump sum fee of$14,000. This does <br /> not include any underwater diving inspections, but URS may recommend such to the City <br /> based on our findings. <br /> Page 1 of 2 <br />
