� � �� <br /> , <br /> AGREEMENT FOR WATER SERVICE <br /> For <br /> Residential Properties Not Frontinq a City Water Main <br /> In exchange for the City of Decatur, Illinois, a municipal corporation (hereinafter referred to as <br /> "City"), allowing water service to be supplied to property located at- <br /> ��� 30 SD S(�1 D �e S � G�'_ , (hereinafter referred to as "the property" and <br /> owned by 1'� i �.1�1 ,�� 1— lU � L �c� (hereinafter referred to as "Owner"), <br /> which property does not front onto an existing water main, the City of Decatur agrees to allow <br /> the property owner to extend a private service subject to the following conditions and <br /> requirements: <br /> 1. The Owner shall provide an accurate legal description and a street address for the property <br /> for which service is being requested. <br /> 2. The property shall be annexed to the City if it is contiguous to the City or when it becomes <br /> contiguous to.the City. At such time that it becomes contiguous to the corporate limits of the <br /> City, the owner of said property shall, within ninety (90) days after receiving notice from the <br /> City, petition to annex to the City. If a petition is not received within the time period, or the <br /> property is not annexed as a result of the petition, water service will be immediately <br /> terminated without further notice. <br /> 3. The Owner must secure the legal and satisfactory property rights for the use through all <br /> intervening property before applying for an interim service from the City, as follows: <br /> CHECK APLICABLE REQUIREMENT(S): <br /> � If the private service will proceed through or on private property not owned by Owner, the Owner has <br /> the full responsibility to obtain the proper authorization from the intervening property owner(s)that <br /> guarantees the right to install and maintain a water service through the intervening private property <br /> and provide such proof to the City prior to extending a private water service to the property. <br /> Documentation submitted: Yes <br /> � If the private service will proceed through, on or within public rights-of-way for which the City has <br /> jurisdictional control, the Owner shall be subject to the requirements similar to those placed on other <br /> utilities located in the City's rights-of-way and the approval of the City's Director of Engineering & <br /> Infrastructure. <br /> Authorization is hereby given to install a water service in the following street(s): <br /> Name of Street(s) <br /> Director of Engineering & Infrastructure Date <br /> Page 1 <br />
