Attachment A to Work Order No. 1 <br /> Mound Road over Steven Creek Bridge Rehabilitation (SN 058-3062) <br /> Scone of Services: <br /> URS Corporation will provide engineering services to prepare rehabilitation plans and <br /> specifications for the bridge carrying Mound Road over Stevens Creek(SN 058-3062) in Decatur, <br /> IL. <br /> Work will include the following: <br /> (1) Three(3) inspections of the existing structure to monitor the condition of the PPC Deck <br /> Beam superstructure. The inspections will occur at six (6)month intervals with the first <br /> inspection taking place within thirty(30) days of authorization to proceed. In the initial <br /> inspection, the bottom of the existing beams in spans 1 and 3 will be sounded using a <br /> hammer and delaminated concrete will be removed to expose strands for inspection. The 7th <br /> beam from the north in the east span will be looked at more thoroughly. The proper NBIS <br /> forms will be completed. No special equipment will be used and therefore the underside of <br /> span 2 will not be accessible for sounding. <br /> (2) Perform survey to obtain roadway profile, roadway cross sections at approaches, bridge seat <br /> elevations and stream cross-sections. <br /> (3) Prepare and submit a hydraulic report to IDOT. <br /> (4) Prepare plans far a PPC Deck Beam superstructure using 21" deep beams. URS will provide <br /> the City with an estimated cost for both a concrete wearing surface and an asphalt wearing <br /> surface with waterproofing membrane. Raised sidewalks with railing are to be provided on <br /> each side of the new superstructure. If it is determined that a barrier needs to be provided <br /> between the roadway and the sidewalk, and providing such would increase the bridge width, <br /> sidewalk will be provided on only one side. It is anticipated that there will be only minor <br /> adjustments to the profile grade considering the thickness of the proposed overlay. URS will <br /> exam the feasibility of staging construction to allow one lane to remain open during <br /> construction. The need for approach guardrail will be evaluated. <br /> (5) Develop repair details for portions of the substructure. Only minor patching repairs are <br /> anticipated for the substructure units. <br /> (6) Prepare special provisions, estimate of cost, and estimate of time. <br /> (7) Submit prefinal and final plans to the City of Decatur and IDOT, if required. <br /> (8) Review shop drawings. <br /> 1 of 2 <br />
