PLAN DOCUMENT AMENDMENT <br /> EFFECTIVE JANUARY 1, 2006 <br /> This Amendment is intended to allow an increase in the salary redistribution for medical <br /> expenses. <br /> Article III Sa1ary Redirection 3.1 is amended to: "However, in no event shall a participant's <br /> salary redirection exceed$4500 per Plan Year for medical expenses". <br /> This Amendment is intended to bring the City of Decatur Fle�ble Benefit Plan <br /> (the "Plan") into compliance with the recent modification by the Internal Revenue <br /> Service to Prop. Treas. Reg. Sections 1.125-1 and 1.125-2 whereby the employer <br /> may provide for a grace period immediately following the end of the plan year. <br /> The Plan is hereby amended to state that a grace period of no more than two (2) months and <br /> fifteen (15) days is added to the Plan to use the benefits or contributions for a plan year before <br /> those amounts are forfeited under the"use it or lose it"rule. <br /> All other terms and conditions of the Plan that are not affected by this amendment <br /> remain unchanged. <br /> �' � 2r�D3� <br /> Signature: Date. ~ ^ �� <br /> Title: Risk anager � <br />
