, <br /> �,NFARES AND SUBSIDIES <br /> The City shall reimburse the Cab Company for the amount of fare subsidies and non-cash fare <br /> payments based on the fares and fare subsidy amounts recorded on the Cab Ride Forms. Fares . <br /> and fare subsidies under Operation Uplift shall be governed by the following conditions: <br /> �1. The maximum fare charged to Operation Uplift participants by the Cab Company for <br /> each one-way trip shall be limited to, and shall be made on the basis of the following: <br /> �A. If one or more riders are traveling from a single origin to a single destination, <br /> then only one trip shall be charged. <br /> �'1 C. If two or more riders are traveling from a single origin to two or more destin- <br /> ations,then only one trip shall be charged, from the single origin to the destination <br /> of the last rider(s). <br /> �D. If riders are picked-up at two or more origins, and are traveling to one or more <br /> destinations,then only one trip shall be charged, from the origin of the first <br /> rider(s)to the destination of the last rider(s). <br /> NOTE: If any one rider wishes travel to more than one destination, then travel to <br /> each destination is a separate trip and each trip, to be eligible for reimbursement, <br /> must have been scheduled at least the day before the day of service, unless it is an <br /> emergency situation or otherwise approved by the City. <br /> Any change or alteration in the fare schedule for Operation Uplift participants shall <br /> require prior concurrence of the City in writing. <br /> �2. The Cab Company shall collect, for each one-way ride, $1.50 from each eligible <br /> Operation Uplift rider and$1.50 from each escort, five (5) years of age and older, <br /> accompanying an eligible Operation Uplift participant. Personal Care Attendants shall <br /> ride for free. Escorts accompanying an eligible Operation Uplift participant must pay <br /> their portion of the fare in cash.
