Lacations and color photos of points where smoke is observed escaping from pipe. <br /> Evaluation of probable cause. <br /> ADDITIONAL AND/OR EMERGENCY INSPECTION/SURVEYS <br /> We are prepared to offer our services to the City of Decah�r in the inspection and <br /> evaluarion of their sanitary and storm sewer in&astructure by doing whatever it takes to <br /> provide a quality ser�ice in a professianal, safe and cost-effective manner. We have and <br /> will stay focused on the City of Decatur engineering needs;emphasizing long-tenn, <br /> mutuatly beneficial relationships based on trust and respect. <br /> l. T.V. Inspection/Survey <br /> 2. Proper documenta.tion and preparation of all field data. <br /> 3. Evaluation of data and recommendarions of repair of problem areas. <br /> 4. Field engineering surveys for the location of]ost or buried manholes <br /> including horizontal and vertical locations of relocated sewer lines of <br /> which there are no records available. <br /> 5. Emergency work is offered on a 24 hour per day, 7 days per week, with <br /> no more than a two-hour response tirue. <br /> ASSUMPTTONS/CONDITIONS <br /> This Agreement is subject to the following assumptions/conditions: <br /> SCHEDULE <br /> It is anricipated that the T.V. Inspection survey will take place from January 1,201b through <br /> December 31,2018. Any addirional footage to be televised or smoke tasting, manhole <br /> evaluations and other services we are asked to perform will be completed in a timely manner <br /> as agreed to by the City Engineer. The prices in this cantract will be applicable through <br /> December 31,2018. <br /> ACCEPTANCE OF CONDTI'IONS STATEMENT <br /> Tele Scaq Inc.herehy affirms that they are in full acceptance of all conditions and requirements <br /> and are prepared to execute an agreement with the City of Decatur and perform these services <br /> accordingly. <br /> l..
