; ' <br /> HOME PROGRAM <br /> SUPPORTIVE HOUSING AGREEMENT <br /> THIS AGREEMENT, entered into on the /� day of� �j�"�Y ,�'L004 by and between <br /> the City of Decatur, Illinois,hereinafter called the "City", and Dove, Inc., hereinafter called <br /> "Dove", organized under the laws of the State of Illinois. <br /> WHEREAS, the City has entered into an agreement with the U.S. Department of Housing and <br /> Urban Development, hereinafter called "HUD", for the purpose of conducting a HOME Program, <br /> and; whereas the City may commit HOME funds for supportive housing projects; and whereas ' <br /> Dove submitted a proposal dated November 3, 2003 to conduct activities eligible under the I <br /> HOME program guidelines and requested a grant of HOME funds not to exceed 50 000 in ' <br /> HOME funds to conduct activities under the stated proposal. <br /> NOW, THEREFORE, the parties in consideration thereto mutually agree and covenant as <br /> follows: <br /> I. SCOPE OF SERVICE <br /> A. Dove shall use the funds as outlined in the project description attached hereto as <br /> Appendix A, for eligible costs of predevelopment, fees, construction and other eligible <br /> items associated with the development of Harbor Place, eight units of supportive rental <br /> housing to be occupied by female headed households that are affected by substance use <br /> disorders. All additional sources of funds for the project must be confirmed prior to the <br /> start of the project. <br /> B. Dove may receive up to 50 000 from the City under the HOME Program to <br /> implement the proposed activities. Funding will be dispersed based upon actual project <br /> expenditures. <br /> C. Dove shall not obligate or expend any funds under the tertns of this agreement <br /> until a written Notice to Proceed has been issued by the City. <br /> II. DURATION <br /> The term of this agreement and the provisions herein shall end two years from the date of <br /> this agreement unless an extension is granted by the City. A request for any extension to <br /> this term of agreement shall be made in writing and shall include an explanation for why <br /> it is required. The City shall inform Dove within three weeks of the receipt of the request <br /> for extension as to whether it has been granted. <br /> 1 <br />
