Anderson Plaza Decatur, IL 62523 Re: Supplement No. 3 Sediment Basin Project HLC No. 4572.03 ��:`,•� �ct'`�c. �g�� Dear Keith: We are herein requesting Supplement No. 3 to our existing contract for the construction observation work completed at the Mobilization and Sediment Basin sites in 2003. Our records show the completion of all construction work was July 12, 2003. This supplement is for additional work performed by our staff due to the contract being extended beyond the anticipated completion date of fa112C102. We informed City of Decatur staff of the need for this su�plerrient during this period of the project. This supplement has been in process for the past yeax and has been held from being submitted due to the need to close out various construction issues on the project. These issues dealt primarily with completing the observation work, the disposal of tree debris from the project site, Contractor authorizations, erosion control issues at the Bolt property, construction of the mobilization site access road, preparation of final record drawings,required closeout meetings with pernutting and local agencies having juris- diction over the sedimentation site and final record transfers to the City of Decatur. These issues are now complete and for this reason, this request is being made at this time. The supplement covers additional costs incurred during 2003 by Homer L. Chastain & Associates (HLC) and our sub-consultants Hanson Professional Services, SKS Engineers, Dredging Specialists and Architechiral Expressions. Our contract for this work had anticipated all work for the project being completed in late 2002 with the closeout of project documentation to take place in early 2003. The actual time frame for the additional work performed by HLC occurred from mid February through the end of October 2003. Our staff was used on site through July to ensure the construction work was performed to the lines, grades and specifications of the contract documents with tinal docuinentation of all work completed in August through October. HLC continued to invoice the City of Decatur during this time frame to keep City staff aware of what costs would be necessary for this supplement. All charges to the City of Decatur ended as of October 31, 2003 and were included in our Fi�al Invotce ivo. 15
