,� ., <br /> HOME PROGRAM <br /> SUPPORTIVE HOUSING AGREEMENT <br /> �,� I <br /> THIS AGREEMENT, entered into on the �,� `� day of �/i , 2003 by and between <br /> the City of Decatur, Illinois,hereinafter called the "City", and Heri age Behavioral Health Center, <br /> Inc., hereinafter called"Heritage, Inc.", organized under the laws of the State of Illinois. <br /> WHEREAS, the City has entered into an agreement with the U.S. Department of Housing and <br /> Urban Development,hereinafter called "HUD", for the purpose of conducting a HOME Program, <br /> and; whereas the City may commit HOME funds for supportive housing proj ects; and whereas <br /> Heritage, Inc. submitted a proposal dated June 28, 2002 to conduct activities eligible under the <br /> HOME program guidelines and requested a grant of HOME funds not to exceed 50 000 in <br /> HOME funds to conduct activities under the stated proposal. <br /> NOW, THEREFORE,the parties in consideration thereto mutually agree and convenant as <br /> follows: <br /> I. SCOPE OF SERVICE <br /> A. Heritage, Inc. shall use the funds as outlined in the proposal dated June 28, 2002 for <br /> eligible costs of predevelopment, fees, construction and other eligible items associated <br /> with the development of eighteen units of supportive rental housing to be occupied by <br /> adult individuals with a serious and persistent mental illness. All additional sources of <br /> funds for the project must be confirmed prior to the start of the project. <br /> B. Heritage, Inc. mayreceive up to 50 000 ($17,000 from FY2001 and$33,000 from <br /> FY2003) from the City under the HOME Program to implement the proposed activities. <br /> Funding will be dispersed based upon actual project expenditures. <br /> C. Heritage, Inc. shall not obligate or expend any funds under the terms of this agreement <br /> until a written Notice to Proceed is issued by the City. <br /> II. DURATION <br /> The term of this agreement and the provisions herein shall end two years from the date of <br /> this agreement unless an extension is granted by the City. A request for any extension to <br /> this term of agreement shall be made in writing and shall include an explanation for why <br /> it is required. The City shall inform Heritage, Inc. within three weeks of the receipt of the <br /> request for extension as to whether it has been granted. <br /> 1 <br />
