3.9 Insurance and Hold Harmless Provisions. ' <br /> 3.9.1 Insurance Requirements. , <br /> CONSULTANT shall procure and maintain insurance, in the amounts set forth ' <br /> below, for protection from claims under worker's compensation acts, claims for <br /> damages because of bodily injury including personal injury, sickness or disease <br /> or death of any and all employees or of any person other than such employees, � <br /> and from claims or damages because of injury to or destruction of property <br /> includin loss of use resultin therefrom. <br /> 9 9 <br /> The CITY shall be included as an additional insured, and will be provided thirty <br /> (30) days written notice of cancellation, non-renewal or substantial coverage <br /> revision. <br /> COVERAGES SCHEDULE MINIMUMS <br /> Workers' Compensation Florida $100,000 B each accident <br /> Statutory Coverage and Employer's Liability $100,000 B each employee <br /> includin a ro riate Federal Acts $500,000 B olic limit for disease <br /> Comprehensive General Liability $1,000,000 B bodily injury each occurrence <br /> $1,000,000 B bodily injury aggregate <br /> $1,000,000 B property damage each <br /> occurrence <br /> $1,000,000 B ro ert dama e a re ate <br /> Products B Com leted O erations $1,000,000 B a re ate <br /> Business Auto Liability Same as Comprehensive General Liability <br /> All autos owned, hired, or used <br /> Professional Liabilit Same as Com rehensive General Liabilit <br /> Excess or Umbrella Liabilit O tional <br /> An insurer holding a current certificate of authority pursuant to Florida Statutes <br /> shall write insurance. Prior to commencing any work on the agreement or <br /> contract, certificates of insurance, approved by the CITY, evidencing the <br /> maintenance of said insurance should be furnished to the project manager. The <br /> certificates shall provide that no material alteration or cancellation, including <br /> expiration and non-renewal, shall be effective until thirty (30) days after receipt of <br /> written notice by the CITY. All coverage shall name the CITY as an additional <br /> insured. <br /> Receipt of certificates or other documents of insurance or policies or copies of <br /> policies by the CITY, or by any of its representatives, which indicate less <br /> coverage than required will not constitute a waiver of the CONSULTANT'S <br /> obligation to fulfill the insurance requirements herein <br /> CONTRACT <br /> PAG E 7 <br />
