Cresthaven Park Subdivision <br /> Proposed Reimbursement Guidelines and Requirements for the Disconnection of Sump Pumps from the <br /> Sanitary Sewer and the Connection of Sump Pump Discharge into the Cresthaven Storm Sewer System <br /> Dated: June 13, 2003 <br /> The Director of Engineering and Infrastructure has set forth the following requirements governing <br /> the disbursement of reimbursements: <br /> l. General: The City is offering reimbursement for sump pump drain line work in the Cresthaven <br /> Park subdivision. Eligible work includes: <br /> a. The disconnection of sump pump drainage from the sanitary sewer. <br /> b. Sump pump drain line extensions that connect to the City's storm sewer system or the City <br /> street for connection to the proposed sump pump drain line. <br /> Contact City Engineering(424-2747) if there are any questions regarding the eligibility of the <br /> drain line improvement work proposed for your property. <br /> 2. Reimbursement: Reimbursement will be 50% of the eligible costs up to a maximum <br /> reimbursement of$500.00 per property. To be eligible for reimbursement the property must be <br /> within the Cresthaven Park subdivision. <br /> 3. Inspection Required: All sump pump drain lines must be inspected and approved by the City <br /> Plumbing Inspector. The inspection must verify that there is no connection to the sanitary sewer. <br /> 4. Permits Required: All permit fees will be waived for this work. <br /> a. All work on private property will require a City Plumbing Permit. <br /> b. All work on City right of way will require an excavation permit issued by City Engineering <br /> and must be completed by a licensed and bonded contractor. <br /> c. All connections to an existing storm drain pipe(including the proposed sump pump drain <br /> line), storm manhole, or storm inlet will require a tapping permit. Note: All drain lines <br /> extended to the curb line prior to the proposed proiect will be connected bv the City at no <br /> cost to the homeowner. <br /> 5. Reimbursement: The homeowner must submit copies of all receipts for the completed work(or <br /> for materials only, if the work is completed by the homeowner). Receipts must be accompanied by <br /> the Request for Reimbursement form. <br /> a. If a contractor is used, he/she must be a licensed plumber with the City. <br /> b. The City will not reimburse for homeowner time or tools purchased for the project. Tool <br /> or equipment rental is reimbursable if provided by a licensed equipment rental agency <br />, within the City limits. <br /> 6. Completion Deadlines: <br /> a. In order to be reimbursed for work or materials required to disconnect a sump pump drain <br /> line from the sanitary sewer, the work must be verified by the City Plumbing Inspector to <br /> have been completed between December l, 2002 and Se tember 12, 2003. <br /> P <br /> b. Reimbursement for work or materials required to extend a sump pump drain line to <br /> connect to the proposed City sump pump drain line,receipts must be received within 30 <br /> days of the resident's being notified,by City Engineering, of the project's substantial <br /> completion. <br /> c. City reimbursement of approved expenses will be made within 15 working days after the <br /> Request for Reimbursement has been submitted. <br />
