(Bodine) is
pleased to provide the enclosed Service Agreement for conducting a Phase II - Limited
Subsurface Investigation for the above-referenced site. Reference for this proposal is provided in
"Phase I Environmental Site Assessment of Mound Middle School and Sunnyside Elementary
School, 3789 North Water Street and 303 West Mound Road, respectively, Decatur, Illinois,
62526, "Bodine Errvironmental Services, Inc., January 2003.
Prepazation of the Phase II- Limited Subsurface Investigation required conducting a visit to the
Decatur Public School District to verify the locations of the former Underground Storage Tanks
(USTs)from the 1966 and 1956 design drawings. There were no "as-built drawings"and the
actual locations of the USTs may not be consistent with the design drawings. From the design
drawings, the former 5,000 gallon Heating Oil UST is indicated to be within the existing Mound
Middle School building. Therefore, Bodine will attempt to utilize equipment capable of boring
down to approximately ten(10)feet below ground surface inside the building.
In conversations with the City, it was indicated that the investigation should be planned to
address the potential for seeking a No Further Remediation Letter from the Illinois
Environmental Protection Agency(Illinois EPA). Therefore, the number and proposed locations
of the borings for the USTs are consistent with the current Illinois EPA Leaking Underground
Storage Tank(LUSTProgram requirements. As such, the proposed borings and samples for the
USTs exceed the number of borings typically utilized to confirm the potential for release under a
Phase II Investigation.
5350 East Firehoese Road Decatur, Illinois 62521-9601 ■ 800/637-"t37° ■ FAX: 2 1 7/8 642 08 6
