(Bodine) is <br /> pleased to provide the enclosed Service Agreement for conducting a Phase II - Limited <br /> Subsurface Investigation for the above-referenced site. Reference for this proposal is provided in <br /> "Phase I Environmental Site Assessment of Mound Middle Schood and Sunnyside Elementary <br /> School, 3789 North Water Street and 303 West Mound Road, respectively, Decatur, Illinois, <br /> 62526, "Bodine Environmental Serviees, Inc., January 2003. <br /> Prepaxation of the Phase II -Limited Subsurface Investigation required conducting a visit to the <br /> Decatur Public School District to verify the locations of the former Underground Storage Tanks <br /> (USTs)from the 1966 and 1956 design drawings. There were no"as-built drawings"and the <br /> actual locations of the USTs may not be consistent with the design drawings. From the design <br /> drawings,the former 5,000 gallon Heating Oil UST is indicated to be within the existing Mound <br /> Middle School building. Therefore, Bodine will attempt to utilize equipment capable of boring <br /> down to approximately ten(10)feet below ground surface inside the building. <br /> In conversations with the City,it was indicated that the investigation should be planned to <br /> address the potential for seeking a No Further Remediation Letter from the Illinois <br /> Environmental Protection Agency(Illinois EPA). Therefore,the number and proposed locations <br /> of the borings for the USTs are consistent with the current Illinois EPA Leaking Underground <br /> Storage Tank(LUST)Program requirements.
