AGREEMENT FOR <br /> PHASE II -LIMITED SUBSURFACE INVESTIGATION <br /> BETWEEN <br /> CITY OF DECATUR <br /> AND <br /> BODINE ENVIRONMENTAL SERVICES, INC. <br /> Bodine Environmental Services, Inc.; 5350 East Firehouse Road, Decatur, Illinois 62521; called <br /> Bodine and City of Decatur, 1 Gary Anderson Plaza, Decatur, Illinois called Client, agree that <br /> Bodine shall provide the services described in Article 1.0, 2.0 and 3.0 in accordance with <br /> applicable laws, rules and regulations and in accordance with and subject to the terms and <br /> conditions set forth herein. <br /> The project scope consists of initiating and completing for the Client investigation activities and <br /> preparation of a letter report for the Mound Middle School and Sunnyside School, Parcel ID# 07- <br /> 34-226-001. The objective of the project is to perform soil borings, collect soil samples, conduct <br /> laboratory analysis of the samples and prepare a letter report summarizing the concentrations of <br /> contaminants detected, if any, against Client specified remediation or cleanup objectives, for the <br /> aforementioned property. <br /> The reference for the Phase II Investigation is the "Phase I Environmental Site Assessment of <br /> Mound Middle School and Sunnyside School, 3789 North Water Street and 303 West Mound <br /> Road, Decatur, Illinois 62526,"Bodine Environmental Services, Inc., June 2003. From the <br /> Phase I,two former underground storage tanks were identified. Bodine was unable to obtain <br /> information regarding reported releases, registrations with the Office of State Fire Marshal ar <br /> formal documentation of the removal of the USTs through various sources. In addition, no <br /> laboratory analytical data was available to assess the potential for a release or comparison to the <br /> Illinois EPA Soil Remediation Objectives in 35 Illinois Administrative Code 742. �I <br /> 1.0 FORMER UST LOCATION INVESTIGATION <br /> 1.1 The Client will obtain access from the current property owner/agent. <br /> 1.2 Bodine will contact the Joint Utility Locating Information for Excavators(JULIE), or <br /> similar State-specific locating service, and work with the facility owner to locate <br /> underground utilities not identified by the locating service prior to initiation of the <br /> subsurface investigation activities. <br /> -1- <br /> BODINE ENVIRONMENTAL SERVICES, INC. <br />
