. <br /> . � <br /> FY 03/04 CDBG <br /> SUBRECIPIENT AGREEMENT <br /> THIS AGREEMENT, entered this 13th day of June , 2003 by and between the <br /> City of Decatur, Illinois (herein called the "City") and Webster-Cantrell Hall <br /> (herein called the "Subrecipient"). <br /> WHEREAS, the Grantee has applied for and received funds from the United States Government <br /> the U.S. Department of Housing and Urban Development ("HLTD") under Title I of the Housing <br /> and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS, the City wishes to engage the Subrecipient to assist the City in utilizing such funds <br /> in accordance with HUD idelines and Federal re lations• <br /> � � � <br /> NOW, THEREFORE, it is agreed between the parties hereto that; <br /> I. STATEMENT OF WORK: <br /> A. Activities: II <br /> The Subrecipient will be responsible for administering a CDBG Year 2003-2004 <br /> "Building on Success" in a manner satisfactory to the City and consistent with any <br /> standards required as a condition of providing these funds. Such program will <br /> include the following activities eligible under the Community Development Block <br /> Grant Program: <br /> Program Delivery <br /> Activity#1 <br /> Funding will be used to renovate the facility's exterior fa�ade in accordance with the <br /> Davis-Bacon Act (see page 12 paragraph (3.) Labor Standards). This facility <br /> provides dormitory housing accommodations and program services to abused and <br /> neglected children awaiting adoption or family reunification. Improvements will <br /> perserve the longevity of the physical structure which will reduce future maintenance <br /> requirements. <br /> General Administration <br /> The Program Coordinator will be responsible for the oversight of this project. All <br /> financial and performance reports will be processed by the Program Assistant. <br /> B. Levels of Accomplishment- Performance Goals <br /> Subrecipient Agreement(Rev.
