Anderson Plaza <br /> Decatur, Illinois 62523-1196 <br /> Subject: Proposal for Engineering Services <br /> 2002 Annual Inspection of the Lake Decatur Dam <br /> Dear Mr. Berard: <br /> As requested in your facsimile dated September 26, 2002, MWH is pleased to submit this proposal <br /> for engineering services to conduct the 2002 Annual Inspection of the Lake Decatur Dam. The <br /> � inspection will be conducted and the report prepared in accordance with the general requirements of <br /> the Illinois Department of Natural Resources,Office of Water Resources. <br /> Scoue of Services <br /> Visual inspection of the Lake Decatur Dam will be conducted in accordance with the Illinois <br /> Department of Natural Resources, Office of Water Resources, procedures and guidelines. The <br /> inspection will be performed by Mr. Mario Finis and Ms. Elizabeth Grom. <br /> MWH proposes to conduct the inspection in late October or early November, depending on the <br /> availability of City personnel. We anticipate that the inspection will be conducted in one day. MWH <br /> will review reference data prior to the inspection, and will conduct a brief ineeting with dam <br /> personnel to discuss any changes in dam operation and maintenance over the past year. We <br /> recommend that the stilling basin be dewatered for the inspection. This would serve a dual purpose to <br /> allow access to the downstream face of the dam and accommodate visual inspection of the stilling <br /> basin endsill and riprap placement. We assume that someone from the City will be available to assist <br /> us during the inspection. <br /> Illinois Department of Transportation - Division of Water Resources inspection reports are required <br /> for the inspection of the dam. A letter report for the inspection of the dam will be prepared. This <br /> report will include a description of the project site, an explanation of the inspection procedures and <br /> observations, an evaluation of operation and maintenance procedures, a description of downstream <br /> development and observations, and conclusions and recommendations. We anticipate that the <br /> description of the project site and historical information of the project can be obtained from the <br /> previous annual inspection reports prepared by MWH (formerly Harza). In addition to the letter <br /> report, appendices will be attached to the report. The appendices will include the dam inspection <br /> checklist forms required by the Division of Water Resources, and photographs reinfarcing our <br /> inspection observations. ' <br /> MWH will submit one copy of the draft report to the City for review and comment approximately <br /> three weeks after completion of the inspection.
