REQUEST FOR PROPOSAL • & CHANGE ORDER Date: 4-03-03 RFP Number: 4A Revisions to Dredge Mobilization Site access road vertical alignment, riprap ditches, excess earthwork disposal, removal of access road and work area lime stabilization, and addition of filter fabric at access road and work area To: Koester Contracting Corporation City of Decatur Project 9956 14649 Highway 41 North Lake Decatur Basin 6 Sediment Storage Evansville, IN 47725 Facility and Dredge Mobilization Site Construction Agreement Date: 08-20-2002 The City of Decatur contemplates making certain changes, additions, and deletions to the work to be performed under the subject Agreement. Unless otherwise indicated in the description of change, accompanying drawings and specifications, all work shall conform to the contract documents. The Contractor is required to submit within 14 calendar days from the date herein a proposal and a detailed breakdown for this change. • REASON for change: The original design proposal resulted in a need to haul approximately 9,000 cy of excess earth excavafion maferial from the mobilizafion site to the sediment sforage facilify. Value engineering studies indicate that a change in fhe profile grade for the access road and reducing fhe amounf of excavation for the difches, along with fhe City's permission to use an area south of fhe east end of the access road for the disposal of excess earth excavafion material, will make it possible fo eliminate the need to haul excess material off-site. During consfrucfion it was found fhat the clay sub-base soils compacfed very well and the need for lime stabilization could be deleted. Filfer fabric was specified for placemenf under fhe aggregate roadway course. DESCRIPTION of change including reference to drawings and specifications revised, new drawings and specifications issued: The atfached revised Plan Sheefs Nos. C1-1, C1-2, C2-1, C2-2, & C2-3;and revised Cross-section Sheets Nos.X-1 through X-13 detail the proposed revisions to be made. The major work items affecfed by the revisions are tree removal, earth excavation, earth embankmenf, A-3 riprap ditch, filter fabric for riprap difches, increased pipe culvert lengfhs,seeding, lime stabilization, and filfer fabric for aggregate surface course. ,,ti SUB-CONTRACTORS affected by this change: r • `r.;.
